What is SPRAVATO?

SPRAVATO® is a prescription medicine used:

• With or without an antidepressant taken by mouth, to treat adults with treatment-resistant depression (TRD).

• With an antidepressant taken by mouth, to treat depressive symptoms in adults with major depressive disorder (MDD) with suicidal thoughts or actions.

SPRAVATO is not for use as a medicine to prevent or relieve pain (anesthetic).
It is not known if SPRAVATO is safe and effective as an anesthetic medicine.

How Spravato Works

Spravato contains esketamine, a derivative of ketamine, which is administered as a nasal spray under medical supervision. Unlike traditional antidepressants that target serotonin, Spravato works on the glutamate system—the brain’s most abundant neurotransmitter.

• Mechanism of Action: Esketamine is an NMDA receptor antagonist, which enhances synaptic plasticity and promotes the growth of new neural connections. This rapid-acting mechanism allows patients to experience a significant reduction in depressive symptoms, often within hours or days—compared to weeks for conventional medications.

• Why It’s Effective: By targeting the glutamatergic system, Spravato bypasses the delayed serotonin-based pathways and directly influences the circuits involved in mood, cognition, and stress response.

Who Is Spravato For?

Spravato is FDA-approved for:

1. Treatment-Resistant Depression (TRD) – Patients who haven’t responded adequately to at least two different oral antidepressants.

2. Major Depressive Disorder with Suicidal Ideation or Behavior – Providing fast-acting relief in urgent clinical scenarios.

Spravato is used in conjunction with an oral antidepressant and is not a standalone treatment.

What to Expect During Treatment

• Administration: Spravato is self-administered intranasally at the clinic under the supervision of a trained provider. Each session lasts approximately 2 hours, during which patients are monitored for safety and side effects.

• Treatment Schedule:

  • Induction phase: Twice weekly for the first 4 weeks

  • Maintenance phase: Weekly or biweekly, based on clinical response

• Common Side Effects: Dizziness, dissociation, nausea, increased blood pressure, sedation, or a sense of detachment. Most symptoms resolve shortly after treatment.

Is Spravato Safe?

Spravato is only available at REMS-certified clinics, like Balanced Minds Psych, to ensure the highest standards of safety. After each dose, patients are monitored for at least two hours and are not permitted to drive, operate machinery, or travel alone until the next day.
